Patient Reporting Form

These resources have been complied to accompany version 3 of the National Patient Report Form. They provide all the material necessary for running a training session with members or for individual study if members so wish. It may be freely copied for distribution to county staff and divisions within counties.

Version 3 of the form has been developed following extensive consultation with our membership. The format remains the same but the contents and layout have been amended as a result of experience and suggestions from members.

Whilst the form remains A3 in size it will no longer be supplied in a pad format but will now be supplied as an A4 folded version and all this then needs is a card insert before completion. This should also prevent the loss of forms when the insert was not put in place on the pads and enable easier distribution of personal issue forms to members.
